The Y is now ready and able to be an active partner in your health journey.
Thanks to a recent grant from Maricopa County Department of Health and other grant funders, we are now accepting referrals from doctors who are prescribing physical activity, diabetes prevention or blood pressure monitoring, and more.
Many of these services are now free of charge or low cost and financial assistance is available.

Your healthcare doesn’t end when you leave your doctor’s office. It’s only beginning – it continues at the Y.
Because of this grant, the Valley of the Sun YMCA provides whole health evaluations, matching members and patients to programs to help address their medical recommendations and works with your practitioner to put their medical advice into action.
It Starts By Raising Your Hand
We can help guide patients with health issues ranging from diabetes to depression, from chronic disease prevention to hypertension, to obesity to cancer.
Following a self-referral using this form OR a referral from your physician or healthcare provider, you can meet with one of our Community Health Workers to assess your needs and match you to programs like weight loss, strength building, diabetes prevention, and more.
We guide you every step of the way.
It all starts with a free, comprehensive Whole Health Evaluation with one of our certified Community Health Workers.
Once your evaluation is complete, we’ll match you with a program and support you along the way.

How It’s Possible
A Community Effort
Beginning in 2022, the Valley of the Sun YMCA received grants from Maricopa County to lead a community-wide effort to make health programs more accessible.
Due to the Y’s efforts, a Whole Health Initiative was formed to facilitate and support wellness and make health services more convenient, using local Y branches, programs, certified health staff and technology.
The Y has joined with over 20 community partners like Abrazo, Banner Health, Mountain Park Health, Arizona State University Edson College of Nursing and ASU’s College of Health Solutions, Valley of the Sun United Way, the Arizona Department of Health Services, Neighborhood Outreach Access to Health and many others to bring health services to our members and the community.

Programs Available:

- Diabetes Prevention
- Helps adults with prediabetes reduce their risk of developing type 2 diabetes
- Goals include reducing body weight by 7% and increasing physical activity to 150 minutes per week
- Youth & Family Diabetes Prevention
- Family-centered diabetes prevention program that supports lifestyle changes through healthy eating and exercise to make positive changes in the families’ health and wellbeing
- 12-week program for youth and their families ages 8-14 with BMI> or + 85th percentile of age and gender
- Weight Loss Support
- Focuses on sustainable ways to eat healthier, increase physical activity and lose weight
- Groups meets 3 times per week for 12 weeks
- Pedaling for Parkinson’s
- Helps adults diagnosed with Parkinson’s disease reduce symptoms through moderate cycling
- Groups meet on an ongoing basis
- Medical clearance recommended
- Cancer Survivorship and Support
- Strength and fitness program for adult cancer survivors
- Participants meet with a certified instructor to increase flexibility, endurance, build muscle and improve ability to perform daily activities
- 12-week program, meets 2 times per week for 90 minutes
- Medical clearance required
- Nutrition Counseling
- An in-depth look at individualized nutrition to support health behavior changes for improvement of health and well-being
- Includes dietary analysis and nutrition coaching
- Personal Training for Strength, Stability and Recovery
- Work with a trainer to identify your health goals, address barriers to reaching those goals, and create a plan unique to you
- Good option for people not yet ready for a specific program or who are seeking additional support outside of programming
- Cardiovascular Strength
- Fall Prevention
- The program, based on the principles of Tai Chi, is led by a qualified instructor and teaches 8 movements modified especially for fall prevention
- The program works to improve balance, muscle strength, flexibility and mobility to enhance overall physical health, which leads to better functioning in daily activities
- Participation in the program may also result in better mental health, reducing stress, improved memory and cognition, and increased self-esteem
- Social Connection and Wellness
- Physician-referred program that focuses on the whole person
- Could include social connectivity, fall prevention, physical activity or other healthy lifestyle activities
- Health & Wellness Screening
- This screening will allow community health workers to understand an individual’s needs and refer them to community services
- Topics may include individual, family, and community support, education, economic assistance, and physical health just to name a few
- Diabetes Prevention
Some programs may have fees attached. In those cases financial assistance is available.
