In 2022 and 2023, the Valley of the Sun YMCA received multiple grants from Maricopa County Department of Health to lead a community-wide effort to make health programs more accessible. 

We have a team of state-certified and trained Community Health Workers ready and waiting to help support your patients and coordinate care.

Collaboration with Healthcare Providers

Our Y Whole Health Initiative includes 20 community health organizations, including providers and medical clinics, volunteering to facilitate and support wellness and make health services more convenient, using local Y branches, programs, certified health staff and technology. We meet monthly to collaborate, connect and move this health initiative forward – together. If you are interested in joining the task force, CLICK HERE to submit an interest email.

Will You Join Us By Referring Your Patients?


How It Works

Using a secure pathway, you can now refer patients to the YMCA’s health programs for chronic disease support like weight loss, strength building or diabetes prevention, and more.

Once a referral is made through our secure system, our Community Health Workers reach out to your patient, evaluate them, and connect them to programs that address your referral. The CHWs also track their participation and report back to you on progress via our Well secure, HIPAA-compliant system.

This enables ongoing patient care and forms a valuable health partnership.

With direct referrals, individuals are far more likely to participate in health programs and to make progress toward better outcomes.
